***REMINDER*** U15 Conditioning Ice Time Reminder and Contact Tracing Form

A reminder that U15 Conditioning ice times begin this weekend for all players who have REGISTERED & PAID for the 2021-22 season.  There is no cost for players to attend these conditioning ice times but players must be registered and have their registration fee paid before they can attend.  
 
To ensure the safety of players and volunteers during our continued navigation of COVID-19 protocols, we are asking players to complete the following 2021-22 U15 Contact Tracing Form  (Please note, this form is only to be completed once/player).  This will help make the check-in process more efficient for players attending conditioning ice times.
 
If the contact tracing form is not completed for the player prior to arriving at the scheduled ice time, their name and contact information can be added at the designated check-in location.

Arrival is a maximum of 30 minutes prior to the scheduled ice time
